5% Courtesy Discount

Our 5% pay-in-full courtesy discount will now be available ONLY on the day services are provided. For crowns, the 5% discount will be available on the day your crown is inserted.

We will no longer send statements with stickers offering the 5% discount after treatment has been completed. If you would like to take advantage of the discount, please plan to make your payment in full on the applicable day of service.

Our Statements Are Going Digital — With a New Look!

We’re making it easier and more convenient to receive your statement and pay your balance! Our statements will now be delivered electronically via text and/or email and will have a fresh, new look.

When you receive your e-statement, you’ll be able to review your balance and make a secure online payment in just minutes — no account, login, or password required!

Prefer a paper statement? No problem! Patients may opt out of electronic statements by calling the office and then they will continue receiving paper statements by mail.

Going digital also does not change the ways you can pay. You will still have the option to:

  • Pay securely online through your e-statement
  • Mail a check or cash payment to our office
  • Pay by cash, check, or credit/debit card in our office

Please note: Payment plans are not currently eligible for online payments. If you have an established payment plan, please continue to contact our office for payment assistance.

Annual Medical History Updates

To ensure we always have the most accurate and up-to-date information regarding your health, all patients will be required to complete or update their Medical History Form annually.

Your medical history is an important part of providing safe dental care, and we appreciate your cooperation in keeping this information current.

Premedication Protocols

If you have had a knee replacement, hip replacement, heart valve replacement, and/or are immunocompromised, we will require a written letter from your physician or specialist outlining their recommendations regarding antibiotic premedication before dental treatment.

This letter must be on file with our office prior to your dental appointment in order for you to be seen.

If your physician or specialist determines that antibiotic premedication is not necessary, we still ask that their recommendation be provided to us in writing so that we can document it appropriately in your patient record.

Premedication Prescriptions

Going forward, our office will no longer prescribe or refill antibiotics used for premedication prior to dental appointments.

If your physician or specialist recommends antibiotic premedication, the prescription will need to be provided and managed by the physician or specialist who originally recommended or prescribed the medication.

We encourage patients who require premedication to contact their physician or specialist well in advance of their dental appointment to ensure both the appropriate documentation and prescription are in place.

We understand that changes to office procedures can sometimes require a little adjustment, and our team is always happy to answer questions or help you prepare for your upcoming visit.
